• Equipment: Standard computer systems
• Dress Code: Business casual
• Floating: May float to assignments as needed

Position Summary This role supports inpatient case management and discharge planning for patients within Portsmouth Regional Hospital’s acute care setting. The team manages a caseload of 25–40 patients, focusing on care coordination and transitions to long-term care.

• Inpatient discharge planning
• Guardianship and surrogacy coordination
• Utilization management
• Revenue integrity
• Long-term care and family planning support
  
  

Qualifications

• Minimum 2 years recent hospital case management experience
• Active Compact RN license at time of submission
• BLS certification
• Bachelor’s degree in Social Work or higher
• Strong understanding of acute care dynamics
• Prior critical care experience preferred
